At The Park Centre Family Practice, we pride ourselves on providing the highest quality of patient care.

PROCEDURE FEES

Procedures requiring a Nurse and/or Doctor, in most cases will be Bulk Billed.

However, in order to be able to continue to provide these services,

A charge of $40 will be charged to all patients, to cover the cost of consumables.

This is payable on the day.

Specialised Procedures excluded from Bulk Billing

The following procedures will incur a Private Fee:

• Iron infusions
• Ear syringes
• medical reports/paperwork
• pre-employment medicals

Full Payment is required on the day of your Consultation.

Failure to attend your appointment or give sufficient notice of cancellation (2hrs)

will result in a non-attendance fee of $25.00.

Whilst we understand that sometimes things happen and you cannot make your booked appointment we do ask that you please contact our rooms to cancel this appointment. We do require a minimum of 2 hours for cancellations in order for us to ensure that this appointment time can be used by someone else who needs our care. If you do not cancel your booked appointment or do not attend, you will be charged a fee of $40.00 for this missed appointment. It is a requirement of our practice that this fee is paid prior to any further appointments being booked in.

Where possible we will book you in with the same doctor. Unfortunately your preferred doctor may not always be available when you need them. If you cannot wait you can choose to have an appointment with another doctor who will have full access to your notes and then you can choose to return to your preferred doctor.

If you are too unwell to visit your GP please contact our rooms and we will discuss your individual situation with your doctor. Home visits are provided at the discretion of the GP.

We have a customer feedback form and box in our waiting room, however if you would prefer to speak with the practice manager directly please call our practice on 9720 4600 and ask to speak with the manager.
